Our physicians see patients by appointment Monday through Friday. Appointments may be scheduled by calling 662.234.9888.

Our entire staff wishes to make your appointment with us a useful and efficient experience. We do everything possible to protect your valuable time. Should your appointment be delayed, please do not hesitate to ask the receptionist to reschedule you for another time or day.

If you are unable to keep your appointment, please notify us within 24 hours.

First Time Visit

Please arrive about 15 minutes before your appointment time in order to complete necessary paper work.

IF YOU ARE TAKING MEDICATIONS OF ANY KIND, PLEASE BRING YOUR MEDICATIONS WITH YOU.

If you must cancel or change your appointment, please notify our office at lease 24 hours in advance. It is the clinic's policy to charge for any time you may have reserved with the doctor.

Payment is required at the time of service. The clinic accepts personal checks, Visa and Mastercard. If you pay in cash, we will provide you with a cash receipt.

If you would like the clinic to file a claim with your insurance company, please bring your insurance cards. Your bill for services rendered is your responsibility.

If your insurance company requires pre-authorization, please be sure this has been done prior to your appointment. We must have this authorization in the clinic prior to your visit. The clinic does not arrange for pre-authorization.

Workers Compensation cases must be pre-authorized prior to your first appointment. If you are expecting to file under Workers Compensation, please make sure you have discussed this with the scheduling secretary.
