Tests or services not provided by our office are billed separately and not part of our fees.
At the time your appointment is made, you will be asked to provide information to us regarding any insurance coverage you may have. Upon your arrival at our office, we will ask you to verify all personal information and assign insurance benefits, if applicable.
You will be responsible for any co-pays, deductibles and/or co-insurance at the time of your appointment.
